Assessment with report– face to face with individual client (with carer/support worker as required).    The session will identify each client’s specific needs and interests and a written report will be provided to the client for confirmation/agreement to ensure an appropriate program is delivered.  Time allocated one hour with client plus writing report. 

 

Assessment only - As above.  At the end of the session there will be a discussion about the outcomes of the session and the suggested program.

Report writing – written report to review client progress, revise program and develop amended program if required.  Estimated time allocated to write report between 30 minutes and an hour.
